Added ability to pull data set from external SQL server. This can be used to connect to MS SQL and MySQL servers at the moment, but let us know if you want any other types. The MS SQL connection requires the pdo_sqlsrv php plugin.

Added ability to change template and item from URL when using the includes data menu item. To change template pass ditmpl=template_id and for item=item_alias as part of the url.
